Imagine that you run a small software firm. One of your engineers just stopped by for his annual performance review. You ask what he accomplished in the past year and he says: “My new software crashed the systems of half our clients. They lost millions and they’re considering suing us! But you can’t fire me because anyone else you hire would be worse. So you have to keep me.
